We are seeking an experienced and skilled Elderly Care Specialist to join our team at Northumbria Healthcare NHS Foundation Trust. As a key member of our multidisciplinary team, you will be responsible for providing high-quality care to our elderly patients, working closely with medical staff and other healthcare professionals to ensure the best possible outcomes.
Key Responsibilities
- Providing specialist care to elderly patients, including conducting thorough assessments, developing treatment plans, and coordinating care with other healthcare professionals.
- Working autonomously to assess individual patient needs, initiate investigations, and determine differential diagnoses.
- Collaborating with medical staff and other healthcare professionals to ensure seamless care and effective communication.
- Developing and implementing care plans that meet the unique needs of each patient, taking into account their medical history, social circumstances, and personal preferences.
- Providing education and support to patients, families, and carers to promote self-management and independence.
- Participating in quality improvement initiatives to enhance patient care and outcomes.
Person Specification
We are looking for an experienced and skilled Elderly Care Specialist who possesses the following qualities:
- 1st level registered nurse/AHP with substantial experience in elderly care.
- Registered Mentor with a strong track record of supporting and developing others.
- Degree-level study to Masters level or equivalent experience in a relevant field.
- Advanced clinical skills qualification at level 7.
- Non-medical prescribing qualification.
- Demonstrated understanding of expanded scope of professional practice issues.
- Evidence of continuing academic and professional development.
