Career Opportunity

The NHS is looking for a skilled Clinical Assessment and Care Coordinator to join our Mental Health and ABI team in Hampshire and Isle of Wight. In this role, you will be responsible for assessing and approving funding applications for clients requiring care and support.

About the Job

  • You will work closely with healthcare professionals and social services colleagues to ensure that clients receive safe and effective care.
  • Assess and approve funding applications for clients with S117 aftercare entitlement.
  • Promote Personal Health Budgets to encourage independence and timely access to care and support.
  • Develop strong relationships with healthcare professionals and social services colleagues to ensure effective collaboration and communication.

Key Requirements

  • Registered Health or Social Care Professional with current registration (NMC, HCPC, or SWE).
  • Educated to degree level in a relevant subject or equivalent experience working at a similar level in healthcare or social care.
  • Experience in assessment, care planning, and coordination, particularly with autistic individuals, learning disabilities, and mental health issues.
  • Knowledge of complex neurological conditions and experience working with clients with brain injuries.

Compensation and Benefits

This is a full-time role offering a competitive salary of £50,000 - £60,000 per annum, depending on experience, plus benefits including professional development opportunities and a supportive work environment.
