Community Learning Disability Nurse

We are seeking a highly skilled and experienced Community Learning Disability Nurse to join our team at Central and North West London NHS Foundation Trust. As a key member of our Community Learning Disability Health Team, you will be responsible for providing high-quality clinical assessment and intervention services to adults with learning disabilities in the community.
Key Responsibilities- Provide comprehensive support and care to service users, taking into account their health, social, and cultural needs.
- Ensure that clients are consulted and involved in decision-making regarding their care.
- Conduct thorough risk assessments and develop innovative care plans to meet the needs of clients.
- Provide clinical supervision to junior staff and students.
- Maintain accurate and up-to-date clinical records and ensure compliance with Trust policies and procedures.
- Stay up-to-date with relevant legislations and policies, and adhere to best practice guidelines.
- Current Professional Registration and qualification in Nursing (RNLD).
- Evidence of post-registration training and continuing professional development in mandatory and statutory areas, such as safeguarding, Mental Capacity Act, and Mental Health Act.
- Previous experience of working with Learning Disabilities service users, including those with serious and enduring mental health problems and challenging behavior.
- Post-registration experience of working in Learning Disabilities.
- Excellent communication and teamworking skills, with the ability to convey enthusiasm and a positive working attitude.
