About the Role

We are seeking a dedicated and compassionate individual to join our team as a Hospice Community Carer. In this role, you will provide high-quality care and support to patients in their own homes, enabling them to live their final days with dignity and comfort.

Key Responsibilities

  • Deliver personalized care and support to patients in their own homes, promoting independence and dignity.
  • Work collaboratively with the multidisciplinary team to ensure seamless care coordination and effective communication.
  • Provide emotional and practical support to patients and their families, addressing their physical, emotional, and spiritual needs.
  • Develop and maintain strong relationships with patients, families, and healthcare professionals, ensuring a person-centered approach to care.
  • Contribute to the development and implementation of individualized care plans, ensuring that patients' needs are met and their goals are achieved.
  • Participate in ongoing education and training to maintain and enhance your skills and knowledge, ensuring that you remain up-to-date with best practices in end-of-life care.

Requirements

  • Qualifications in care or a related field (e.g., NVQ Level 2 or equivalent).
  • Experience in providing care and support to patients in their own homes or in a hospice setting.
  • Excellent communication and interpersonal skills, with the ability to build strong relationships with patients, families, and healthcare professionals.
  • Ability to work independently and as part of a team, with a flexible approach to working hours and shifts.
  • Current driving license and access to a car.
