About the Role

We are seeking a dedicated and compassionate individual to join our team as a Hospice Community Carer. As a key member of our care team, you will provide high-quality care and support to patients in their own homes, enabling them to live their final days with dignity and comfort.

Key Responsibilities
  • Deliver personalized care and support to patients in their own homes, ensuring their physical, emotional, and spiritual needs are met.
  • Work collaboratively with the care team to develop and implement individualized care plans that meet the unique needs of each patient.
  • Provide emotional support and companionship to patients and their families, promoting a sense of calm and well-being.
  • Assist patients with daily living tasks, such as bathing, dressing, and medication management.
  • Monitor patients' conditions and report any changes or concerns to the care team.
  • Maintain accurate and up-to-date records of patient care and progress.
Requirements
  • Qualifications in care or a related field (e.g., NVQ Level 2 or equivalent).
  • Experience in care or a related field, preferably in a hospice or palliative care setting.
  • Excellent communication and interpersonal skills, with the ability to build rapport with patients, families, and colleagues.
  • Ability to work independently and as part of a team, with a flexible approach to working hours and shifts.
  • Current driving license and access to a car.
