Job Summary

The Hospice of St Francis is seeking a skilled and compassionate Hospice Community Carer to join our team. As a key member of our care team, you will provide high-quality, patient-centered care to individuals in their own homes, focusing on comfort, dignity, and well-being.

Main Responsibilities
  • Deliver personalized care to patients in their homes, ensuring their physical, emotional, and social needs are met.
  • Work collaboratively with the care team to develop and implement individualized care plans.
  • Provide emotional support and companionship to patients and their families, promoting a sense of calm and well-being.
  • Administer medication and perform personal care tasks as required.
  • Document patient progress and report any changes or concerns to the care team.

Requirements
  • Valid Care Certificate or NVQ Level 2 qualification.
  • 3 years' experience in a similar role, with a proven track record of delivering high-quality care.
  • Excellent communication and interpersonal skills, with the ability to build rapport with patients and their families.
  • Ability to work independently and as part of a team, with a flexible approach to working hours.
  • Current driving license and access to a car.
