Job Summary

We are seeking a dedicated and compassionate individual to join our team as a Senior Care Practitioner in our Leeds Extra Care Service. As a key member of our support team, you will be responsible for delivering high-quality person-centred care and support to older adults.

About the Role

The successful candidate will have experience in providing care and support to individuals with varying needs, including learning disabilities and mental health conditions. You will be required to work flexibly to meet the needs of our service users, including evenings, weekends, and bank holidays.

Key Responsibilities
  1. Provide person-centred care and support to older adults in our Leeds Extra Care Service.
  2. Assess the needs of service users and develop support plans that promote independence and well-being.
  3. Work collaboratively with other professionals to ensure seamless delivery of care services.
  4. Maintain accurate records of service user interactions and progress.
