Salary: We offer a competitive compensation package, including a base salary of $85,000 per year, plus bonus pay opportunities.

Job Description: As an Autonomous Clinical Practitioner, you will provide exceptional care to seniors in a private practice setting without back office hassle. You will work closely with our team-oriented staff to deliver personalized care and support.

Required Skills and Qualifications: To be successful in this role, you must have a Master's degree, a valid LISW license in the state, and experience working in nursing home/hospital facilities. Strong medical knowledge, understanding of documentation (CMS, Medicare, Medicaid), and ability to foster open communication with facility teams and Regional Clinical Managers are essential. Passion for working with older adults is also required.
