Job Summary

We are seeking a dedicated Health and Wellbeing Coach to join our team. As a Health and Wellbeing Coach, you will support patients, particularly those with long-term conditions, in managing their health and wellbeing. Using health coaching techniques, you will empower patients to understand and manage their conditions independently.

Main Responsibilities

  • Provide personalised care and wellbeing coaching to a cohort of patients.
  • Work closely with GPs and multidisciplinary teams to develop and implement patient care plans.
  • Participate in multidisciplinary team meetings to discuss patient progress.
  • Engage with community resources and initiatives to promote healthy lifestyles.
  • Manage a caseload independently, ensuring patient outcomes are met.
  • Support quality improvement projects across the organisation.

Person Specification

Essential Skills and Knowledge

  • IT skills.
  • Excellent communication skills and interpersonal skills, including an ability to build rapport and establish good one-to-one relationships quickly.
  • Ability to deal with challenging behaviour.
  • Ability to create and maintain professional boundaries.

Desirable Skills and Knowledge

  • Experience in using databases.
  • Experience of working without direct supervision.
  • Experience in using community-based activities and resources to support individuals.

Essential Experience

  • Evidence of ability to both work autonomously and to seek support and guidance when appropriate.
  • Evidence of achieving targets and proactive approach.

Desirable Experience

  • Evidence of working within a multidisciplinary team.
  • Evidence of target-based work experience.
  • Coaching experience.
  • Experience working with GP surgeries.
  • Experience working with people with low-level mental ill-health.
  • Smoking cessation training (or willingness to train).
  • Experience with alcohol or drugs brief intervention training (or willingness to train).
  • Experience in working with groups (or willing to train).
  • Experience in health and nutrition work with patients (or willingness to train).

Qualifications

  • English and Maths GCSE or equivalent.
