Becoming a Care Coordinator Specialist at South Warwickshire GP Federation means joining a dynamic team dedicated to delivering high-quality care and services to patients. Your responsibilities will encompass a range of tasks, including coordinating and navigating across health and care services, working alongside Health & Wellbeing Coaches to identify patients requiring additional support, and liaising with relevant parties to ensure timely care. You will also be involved in raising awareness of shared decision-making and assisting patients in being prepared for such conversations.\

To excel in this position, you should possess excellent interpersonal and communication skills, with the ability to build trust and rapport with patients, families, and healthcare professionals. Your technical skills should include proficiency in IT systems and packages, including EMIS Web and Docman 10.\
