Patient Care Specialist

As a Reablement and Rehabilitation Assistant, you will play a vital role in supporting patients to regain their independence after illness or admission to hospital. This rewarding role requires strong communication and interpersonal skills, as well as the ability to work independently in the community.
Key responsibilities include providing personal care, including meals, personal hygiene, rehabilitation support and overall reassurance and safety. You will also offer additional support with training if required in areas such as reablement, phlebotomy and holistic support.
The ideal candidate will have a relevant NVQ Level 2/3 or equivalent qualification and potential NHS experience. A driver's licence and access to a car are also essential requirements for this role.
Salary and Benefits:
