Patient Care Coordinator

We are seeking a skilled Patient Care Coordinator to join our team. As a key member of our department, you will play a vital role in delivering high-quality patient care, working closely with our healthcare professionals.
Key Responsibilities- Coordinate patient care, ensuring seamless transitions between departments and services
- Communicate effectively with patients, carers, and families, providing information and guidance as required
- Collaborate with healthcare professionals to develop and implement individualized patient care plans
- NVQ Level 2 or 3 in Health and Social Care or equivalent experience
- Able to work independently, making decisions and taking initiative as required
- Strong analytical and problem-solving skills, with the ability to think critically and creatively
- An estimated salary of £30,000 - £35,000 per annum, depending on experience
