Salary Information

Job Summary

We are seeking a skilled Case Worker to join our Intermediate Care Team, supporting individuals with complex needs and facilitating smooth transitions between services. Your expertise will be instrumental in ensuring seamless care delivery and outstanding outcomes.

Key Responsibilities:
  • Undertake thorough assessments and reviews of individuals in the community.
  • Offer tailored guidance and support to promote individuals' wellbeing and independence.
  • Cultivate strong partnerships with individuals, families, service providers, and healthcare professionals to identify innovative solutions.
  • Collaborate with the team to drive operational priorities forward.
Requirements

To excel in this role, you will need:

  • An empathetic approach when working with individuals facing challenges.
  • Exceptional written and verbal communication skills.
  • The flexibility to adapt to evolving situations and priorities.
  • Excellent organisational skills, including proficiency in IT and record-keeping.
