Job Title: Adult Community Health Manager

Location: Norfolk

We are seeking an experienced healthcare professional to provide leadership and management to the Adult Community Learning Disability Team (CLDT) in the city locality based at County Hall.

The successful candidate will contribute to the operational delivery, strategic planning, and development of the service, ensuring we continue to improve our delivery of care, treatment, and support to adults with learning disabilities and their families.

Main Responsibilities:

  1. Lead a multi-disciplinary team delivering high-quality care, treatment, and support to adults with learning disabilities.
  2. Manage the team, including recruitment, career development, work performance, and evaluation.
  3. Monitor and manage governance of the team and the services they deliver.
  4. Identify service development opportunities and lead on agreed change processes and projects.
  5. Coodinate and support effective clinical practice, procedures, and systems across organisational boundaries.

Essential Requirements:

  • Degree or Diploma in Nursing, Allied Health Professional, Psychology, or Social Work.
  • Experience of managing Integrated Health & Social Care Teams and Services.
  • Relevant experience in the community and acute sector.
  • Good keyboard skills able to use Information Technology and electronic case management systems.

Estimated Salary: £50,000 - £70,000 per annum depending on experience.
