We are seeking an experienced Social Worker to join our Hospital Discharge Team at Charles Hunter Associates.
About The RoleThis role is responsible for providing high-quality support to adults within the hospital discharge process. Key responsibilities will include triaging referrals, managing a caseload in a timely manner, conducting care assessments, and taking on duty roles.
Key Requirements- A minimum of two years' experience as a Social Worker
- A degree in Social Work (Degree/DipSW/COSW)
The ideal candidate will possess excellent communication skills, be able to work in a fast-paced environment, and have a passion for ensuring individuals receive appropriate care.
What We Offer- A competitive salary: £45,000 - £55,000 per annum (depending on experience)
