As a key member of our team, you will be responsible for providing high-quality nursing care to individuals with learning disabilities, working closely with other healthcare professionals to deliver person-centered and recovery-focused care.
Key Responsibilities- Conduct comprehensive nursing assessments and develop individualized care plans to meet the unique needs of service users.
- Provide expert advice and guidance to service users, carers, and other professionals on various health care needs related to learning disabilities.
- Collaborate with the multidisciplinary team to develop and implement health promotion initiatives, reducing health inequalities and improving health outcomes.
- Participate in the administration, carriage, storage, and safe disposal of medication and equipment used in the community, adhering to Trust policies and procedures.
- Take appropriate action in relation to safeguarding vulnerable adults, ensuring their safety and well-being.
- Work autonomously, managing a caseload and delegating tasks to support staff, as agreed through supervision.
