As a key member of our Primary Health Care Team, you will play a vital role in delivering high-quality patient care and contributing to the implementation of local and national strategies.
Key Responsibilities:- Provide clinical and managerial leadership to the nursing team, ensuring person-centred assessment and care planning.
- Assist in the implementation of local and national strategies, working collaboratively with Clinical Team Leaders.
- Manage the day-to-day operations of the team, providing clinical expertise, supervision, and oversight.
- Work collaboratively with the Clinical Team Leaders to ensure the effective delivery of patient care.
- NMC registration and successful attainment of SPQ Specialist Practitioner Qualification in District Nursing or equivalent.
- Current driving license and ability to travel within the community.
- Excellent communication and interpersonal skills, with the ability to work effectively in a team environment.
