We are seeking a highly skilled and experienced Specialist Nurse to join our team in Somerset NHS Foundation Trust. The successful candidate will have a passion for providing high-quality care to children with complex mental health needs.
Main Responsibilities- Work collaboratively with the Named Nurse for Children Looked After to improve the recognition, assessment, and delivery of healthcare for children with additional mental and emotional health needs.
- Undertake statutory health assessments for children with known additional mental and emotional health needs.
- Provide specialist advice and consultation on the care and treatment of Care Leavers to carers and partner agencies.
- Work collaboratively with the Children Looked After Health Service and CAMHS to develop programmes and pathways that support children to make healthy lifestyle choices and optimise their health.
