Job Title: Children Looked After Specialist Nurse

We are seeking a highly skilled and compassionate Children Looked After Specialist Nurse to join our team at Somerset NHS Foundation Trust. As a key member of our team, you will play a vital role in providing specialist care and support to children in care, with a focus on their mental and emotional health.

About the Role

This is an exciting opportunity to work with a dedicated team of professionals who are passionate about improving the lives of children in care. As a Children Looked After Specialist Nurse, you will be responsible for:

  • Undertaking statutory health assessments for children in care, with a focus on those with additional mental and emotional health needs.
  • Working collaboratively with carers and multi-agency partners to progress health assessments, health care plans, and enable a good understanding of the mental and emotional health care needs of the young people in their care.
  • Providing specialist advice and consultation on the care and treatment of care leavers to carers and the wider partner agencies, ensuring a seamless transition into adult mental health services where indicated.
  • Contributing to the development of programmes, pathways, and policies that support children in care to make healthy lifestyle choices and optimize their health.
