About the Role

We are seeking a highly motivated and dedicated individual to join our team as a Dietetic Lifestyle Support Specialist. As a key member of our Community Dietetic Team, you will play a vital role in supporting patients in making healthy lifestyle changes and improving their overall well-being.

Key Responsibilities
  • Provide brief intervention support to patients within GP settings, utilizing Motivational Interviewing (MI) skills to facilitate positive lifestyle change and signpost relevant local services to reduce the risk of diabetes development.
  • Review patient progress and ensure signposting to other relevant support packages in the community, with a focus on 12-month check-ups.
  • Assist the Community Dietetic Team with administrative tasks, including audit, stock checks, and ordering resources and products essential for the smooth delivery of community dietetic services.
  • Carry out routine administrative tasks, such as collecting post from the main office, checking stock of pumps and stationery, and ordering as required.
  • Plan and organize straightforward activities, ensuring effective delivery of community dietetic services.
  • Review dietetic care plans for designated patients, supporting them to optimize their nutritional intake as part of their community dietetic care pathway.

Requirements
  • English or maths GCSE or equivalent
  • Experience working as part of a team
  • Experience working in a community-focused, NHS, or healthcare setting
  • Experience of using Microsoft Windows, Word, PowerPoint, and Outlook packages
Desirable Qualifications and Experience
  • ECDL or equivalent IT qualification
  • Nutrition qualification
  • Experience of group education or teaching
