About the Role

We are seeking a highly motivated and experienced Community Nurse to join our team at NELFT NHS Foundation Trust. As a Community Nurse, you will play a vital role in providing high-quality patient care and support to individuals in their own homes and community settings.

Key Responsibilities
  • Provide holistic health care to patients, families, and carers in their own homes and community settings.
  • Work collaboratively with other healthcare professionals to deliver individualized care plans.
  • Conduct patient assessments and develop care plans in consultation with patients, families, and carers.
  • Provide education and support to patients, families, and carers to promote health and well-being.
  • Participate in multidisciplinary team meetings to discuss patient care and develop strategies for improving patient outcomes.
Requirements
  • Registered Nurse (Part 1) with current NMC registration.
  • Experience of working in a community setting, preferably with a focus on primary care.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and carers.
  • Ability to work autonomously and as part of a team, with a flexible approach to working hours.
  • Basic awareness of IT and IT skills, with the ability to use electronic patient records and other healthcare systems.
