We are seeking a highly skilled and experienced Clinical Lead to join our team at NELFT North East London Foundation Trust. As a Community Matron, you will play a key role in delivering high-quality patient care and contributing to the development of our services surrounding long-term conditions.

The successful candidate will have a minimum of 5 years' experience as a Registered Nurse, with a strong background in community care and a proven track record of leading and managing change. They will be a prescriber, have mentoring experience, and be able to work as an advanced practitioner with expert clinical skills.

Main Responsibilities
  • Manage a caseload of patients with complex health and social care needs, utilizing advanced nursing skills and quality improvement tools to optimize patient outcomes.
  • Work collaboratively with GPs, social services, and third sector organizations to deliver integrated care and prevent unnecessary hospital admissions.
  • Provide clinical supervision and support to colleagues, as well as targeted education and training to develop their skills and knowledge.
  • Engage positively in performance management and participate in the review and development of their Personal Development Plan.
  • Demonstrate effective clinical and professional leadership in the development of services surrounding long-term conditions.

  • Must be a prescriber
  • Must have a minimum of 5 years experience as a Registered Nurse
  • Must have mentoring course
  • Experience of collaborative working
  • Evidence of leading and managing change
  • Experience in managing conflict
  • Ability to understand and interpret clinical data and information
  • Able to work as an advanced practitioner with expert clinical skills
  • Evidence of NHS plan, frameworks and government policies
  • Basic awareness of IT and IT skills
  • Experience with managing patients with long term conditions
  • Evidence of post-registration education, e.g. specialist practitioner, nurse practitioner, post registration courses
