About the Role

We are seeking a highly skilled and experienced Community Mental Health Nurse to join our dynamic team at NELFT NHS Foundation Trust. As a Community Mental Health Nurse, you will play a vital role in providing high-quality care and support to individuals with complex mental health needs.

Key Responsibilities
  • Assess and develop care plans for individuals with complex mental health needs
  • Work collaboratively with multidisciplinary teams to provide comprehensive care and support
  • Deliver evidence-based interventions and therapies to promote recovery and well-being
  • Provide education and support to individuals, families, and carers to promote mental health and well-being
  • Contribute to the development and implementation of mental health services and policies
Requirements
  • Registered Mental Health Nurse with current registration with the NMC
  • Experience of working in a community mental health setting
  • Strong assessment and planning skills
  • Ability to work collaboratively with multidisciplinary teams
  • Excellent communication and interpersonal skills
