About the Role: We are seeking a highly motivated and compassionate individual to join our team as a Community Connect Coordinator. This exciting role will involve working closely with stroke survivors, their families, and community partners to deliver our innovative Community Connect service in Carmarthenshire.

Key Responsibilities:

  • Support new stroke survivors and their carers to live independent lives in the community, promoting their physical and emotional well-being.
  • Provide personalized information, advice, and support to help stroke survivors achieve their desired outcomes.
  • Work collaboratively with healthcare professionals, social care services, and community organizations to ensure seamless support for stroke survivors.
  • Manage a team of volunteers, providing guidance and support to help them make a positive impact in the lives of stroke survivors.
  • Promote stroke-friendly communities, advocating for inclusive and accessible environments that support the needs of stroke survivors.
  • Travel across the locality to connect people to their community and peers, building strong relationships and fostering a sense of belonging.

Requirements: To be successful in this role, you must be able to demonstrate your ability to work independently and as part of a team, with excellent communication and organizational skills. You must also be resident in the UK and have the right to work in the UK.
