Job Summary

We are seeking a highly organized and detail-oriented individual to join our team as an Apprentice Care Navigator. In this role, you will provide administrative support to our clinical team, ensuring seamless patient care and efficient practice operations.

Key Responsibilities
  • Prepare consulting rooms with clinical stationery, test requests, and sample bottles, following established protocols.
  • Close down consultation rooms, secure the building, and perform daily tasks as required.
  • Check pharmacy fridge temperatures, record temperatures, and take necessary actions to maintain a safe environment.
  • Conduct weekly and monthly building checks, ensuring compliance with practice protocols and reporting any issues.
  • Act as a Fire Marshal during emergencies and fire drills.
  • Reconcile evening appointments, care-navigate patients to appropriate healthcare professionals, and ensure accurate data input.
  • Handle patient correspondence, prescriptions, and other paperwork, maintaining confidentiality and adhering to information governance requirements.
  • Update noticeboards with patient information, ensuring accuracy and safety.
  • Resolve patient issues, notify the Practice Manager of complaints, and follow the complaints procedure.
  • Process new patient registrations, promote digital services, and book appointments with the clinical team.
  • Deal with home visits, emergency presentations, and ensure correct appointments are booked.
  • Participate in practice uptake encouragement initiatives and maintain accurate records.
  • Receive visitors and deliveries, ensuring protocol is followed, and maintain the handover diary.
  • Ensure safeguarding and child protection requirements are met, and report any concerns.
  • Scan, code, and file patient information accurately and promptly.
  • Undertake electronic document management tasks as required.
  • Deal with requests for private work or death administration, and conduct yourself in a manner that ensures patient safety.
  • Follow all policy and procedures relating to health and safety, and report any equipment failures or maintenance requirements.
