Qualified Social Worker for Older Persons Team

We are seeking a dedicated and compassionate qualified social worker to join our Older Persons Mental Health Team in Middlesbrough.
This is an excellent opportunity to make a positive impact on the lives of adults aged 65+ with mental health needs, primarily dementia.
Job OverviewIn this key role, you will be responsible for managing complex casework and conducting thorough reviews, including regular face-to-face visits.
You will work closely with healthcare partners to ensure seamless delivery of care and support to our service users.
Responsibilities:- Develop and implement effective care plans tailored to individual needs
- Conduct regular assessments and reviews to monitor progress and adjust plans as needed
- Build strong relationships with service users, their families, and healthcare professionals
- Registration with Social Work England
- UK eligibility and full UK license
- Minimum 2 years post-qualifying experience in social work
- Degree-level or equivalent qualification in Social Work
- A competitive daily rate of £285
