About the Role

We are seeking an experienced Care Coordinator to join our team at Guardian Homecare Limited. As a Care Coordinator, you will play a vital role in delivering high-quality community care services to our clients.

Key Responsibilities
  • Manage care worker rotas to ensure seamless service delivery
  • Build and maintain strong relationships with care workers and clients
  • Provide exceptional customer service and support to clients and their families
  • Collaborate with the care team to ensure effective care planning and delivery
