Job Title: Care Coordinator

We're seeking an experienced Care Coordinator to join our team, responsible for coordinating care services for our clients. As a Care Coordinator, you will be the key point of contact for our clients, ensuring their care needs are met and exceeded.

Key Responsibilities:

  • Coordinate care services for clients, including scheduling and managing care workers
  • Build and maintain relationships with clients, their families, and care workers
  • Monitor and evaluate the quality of care services, making recommendations for improvement
  • Collaborate with other teams to ensure seamless delivery of care services

Requirements:

  • Experience in care coordination or a related field
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Strong organizational and time management skills

Working Hours:

Monday to Friday, 9am - 5pm, with additional on-call duties on a rota basis (paid extra)
