This role is an integral part of the Integrated Discharge Service, supporting patients across the region. The Care Coordinator will work with patients, families, and clinical teams to support their care needs and associated care packages and placements.
Main Responsibilities- Support the onward care and planning of patients on pathways to provide a proactive and efficient service from admission to discharge.
- Sourcing short/long term care packages/placement for a range of personal needs and specialist needs.
- Work efficiently and effectively to release bed capacity within both the hospital and community area settings.
- Provide a high level of progress chasing, facilitation, and coordination to the discharge planning process for all patients.
- Work in partnership with other health and social teams and services across the hospital(s) that are involved in discharge planning or transfer of care for patients to alternative settings.
- Ensure accurate patient records are kept across multiple electronic systems are kept live and up to date.

Key Skills and Qualifications- English and Maths, minimum GCSE level A-C
- Business Administration qualification NVQ level 3 or equivalent experience
- Good knowledge of IT systems including Outlook, Word, and Excel
- Proven ability to work under own initiative
