Key Responsibilities
  • Provide highly skilled and flexible support to individuals with learning disabilities, promoting their independence and well-being.
  • Work closely with the Resource Allocation Manager to develop and implement personalized support plans.
  • Assist with daily living tasks, such as personal care, medication management, and household chores.
  • Support individuals to access community resources, services, and activities that promote their physical and mental health.
  • Collaborate with other healthcare professionals to ensure comprehensive care and support.
Requirements
  • Essential driving skills and access to a vehicle.
  • Ability to work varied shift patterns, including weekends and sleep-ins.
  • Enhanced DBS disclosure required.
