Key Responsibilities

  • Conduct thorough assessments of adult social care needs, identifying risks and developing effective support plans.
  • Work collaboratively with colleagues, partners, and service users to promote independence, wellbeing, and re-enablement.
  • Apply legislation and policies to practice, ensuring compliance with statutory responsibilities.
  • Develop and maintain strong relationships with service users, families, and carers, promoting empowerment and choice.
  • Contribute to the development of the Thrive Agenda, addressing poverty and health inequalities in our community.
