About the Role

We are seeking a highly motivated and empathetic individual to join our team as a Social Prescribing Link Worker. As a key member of our Primary Care Network, you will play a vital role in supporting patients in Northolt to take control of their health and wellbeing.

Main Responsibilities

  • Work in partnership with our existing Social Prescribing Link Worker and practice teams to support patients in need.
  • Develop and maintain relationships with local community groups and statutory services to provide holistic support to patients.
  • Support the development of new community groups and initiatives to promote health and wellbeing.
  • Provide motivational and behavioral techniques to support patients in achieving their goals.

Requirements

  • Demonstrable experience of working in a community development context, adult health and social care, learning support, or public health/health improvement.
  • Excellent communication skills with the ability to build relationships with patients, community groups, and healthcare professionals.
  • Ability to work under pressure and manage varying workloads and changing demands.
  • Relevant experience of General Practice IT systems.
  • Full driving license and access to a car for regular travel across the locality.

Person Specification

  • GCSE grade A* to C (9-4) in English and Maths or equivalent.
  • Demonstrable commitment to professional and personal development.
  • Willingness to work towards further educational qualifications required for the role.
