Job Summary:

We are seeking a dedicated and compassionate Support Practitioner to join our Mobile Homecare Team in Helensburgh. As a key member of our team, you will work closely with vulnerable individuals in their own homes, providing high-quality care and support to enhance their quality of life.

About the Role:

  • Provide person-centered care and support to individuals in their own homes, promoting independence and well-being.
  • Work collaboratively with colleagues to assess individual needs and develop personalized care plans.
  • Contribute to the development and implementation of care strategies to prevent hospital or care home admissions.
  • Support individuals to access community resources and services, promoting social inclusion and community engagement.

Requirements:

  • Experience working with vulnerable populations, preferably in a community care setting.
  • Excellent communication and interpersonal skills, with the ability to build strong relationships with individuals and their families.
  • Ability to work independently and as part of a team, with a flexible approach to working hours.
  • Knowledge of care and support principles, with a commitment to person-centered practice.
