About the Role
As a Support Practitioner, you will be responsible for delivering high-quality support to individuals, working closely with our team to identify and mitigate risks. You will also be involved in planning and delivering support plans, ensuring that individuals receive the best possible care and support.
Key Responsibilities
- Deliver high-quality support to individuals in their own homes
- Identify and mitigate risks, working closely with the team to develop and implement support plans
- Provide driver support, using a company-provided vehicle
- Work collaboratively with the team to achieve positive outcomes for individuals
