Job Summary

We are seeking a skilled and compassionate Support Practitioner to join our Rothesay Responder Service team. As a Support Practitioner, you will play a vital role in providing effective and efficient support to individuals in their own homes, promoting their safety and well-being while reducing the need for emergency services.

Key Responsibilities
  • Provide high-quality support to individuals in their own homes, focusing on their safety and well-being.
  • Work collaboratively with the Rothesay Responder Service team to ensure a coordinated and effective response to individuals' needs.
  • Develop and maintain strong relationships with individuals, their families, and other stakeholders to ensure a person-centered approach.
  • Contribute to the development and implementation of individual support plans, ensuring they are tailored to meet the unique needs of each individual.
  • Participate in regular team meetings and training sessions to stay up-to-date with best practices and service developments.
Requirements

To be successful in this role, you will need:

  • A strong understanding of the principles of person-centered support and the ability to apply them in practice.
  • Excellent communication and interpersonal skills, with the ability to build strong relationships with individuals and stakeholders.
  • Ability to work independently and as part of a team, with a flexible approach to working hours.
  • Basic IT skills and the ability to use digital tools to support your work.
  • Enhanced DBS clearance and a willingness to undergo regular checks.
