Job Summary

We are seeking a skilled and compassionate Support Practitioner to join our Rothesay Responder Service team. As a Support Practitioner, you will play a vital role in providing effective and efficient support to individuals in their own homes, reducing the risk of accidents and hospital admissions.

About the Role

As a Support Practitioner, you will be responsible for delivering high-quality support to individuals, working closely with our team to identify and mitigate risks. You will also be involved in planning and delivering support plans, ensuring that individuals receive the best possible care and support.

Key Responsibilities

  • Deliver high-quality support to individuals in their own homes
  • Identify and mitigate risks, working closely with the team to develop and implement support plans
  • Provide driver support, using a company-provided vehicle
  • Work collaboratively with the team to achieve positive outcomes for individuals

Requirements
