Paramedic - South Warrington Primary Care Network

We are seeking an experienced and enthusiastic paramedic to join our team at Quay Primary Healthcare CIC, working in collaboration with South Warrington Primary Care Network.
Key Responsibilities- Work as part of the multidisciplinary team to deliver and develop the on-the-day patient service in the practice and PCN.
- Accurately triage, assess, and prioritize patients, including face-to-face same-day and telephone triage, demonstrating the use of various techniques to elicit the history of an event/illness.
- Proactively respond to referrals and provide assessment and diagnosis at the first point of contact, according to patients' needs, either in the practice or patient homes.
- Undertake home visits for the practice or PCN.
- Evaluate clinical information from examination and history taking and initiate appropriate treatment, including prescribing medications following national or practice/PCN policy, patient group directives, national and local clinical guidelines, and local care pathways.
- Review the effectiveness of the treatment provided, making changes where necessary to improve patient outcomes.
- Communicate effectively with patients, and where appropriate, family members and their carers, regarding their physical health needs, results, findings, and treatment choices.
- Advise patients on general healthcare and promote self-management, including signposting patients to the PCN's social prescribing service and other community or voluntary services.
- Communicate proactively and effectively with all colleagues across the multidisciplinary team, attending and contributing to meetings as required.
- Support the delivery of enhanced services and other service requirements on behalf of the PCN.
- Ensure delivery of best practice in clinical practice, caseload management, education, research, and audit to achieve corporate PCN and local population objectives.
- Participate in the management of patient complaints when requested to do so and participate in the identification of any necessary learning brought about through incidents and near-miss events.
- Undertake all mandatory training and induction programs.
- Contribute to and embrace the spectrum of clinical governance.
- Maintain a clean, tidy, effective working area.
- Qualified to degree/diploma level in Paramedicine or equivalent experience.
- Registered with the Health and Care Professions Council (HCPC).
- Completed two-year Consolidation of Learning period as a newly qualified paramedic.
- Has a further three years' experience as a band 6 (or equivalent) paramedic.
- Committed to working towards a master's level or equivalent capability in paramedic areas of practice within six months of the commencement of start date/has completed and been signed off formally within the clinical pillar competencies of the paramedic FCP/AP roadmap to practice.
- Framework for Higher Education Qualification (FHEQ) Level 7 or working towards developing Level 7 capability in paramedic areas of practice and, within six months of the commencement, complete the clinical pillar competencies of the Advanced Clinical Practice Framework.
- Demonstrate excellent clinical competence (evidenced through CPD and interview).
- Safeguarding and other mandatory training.
- Basic life support training.
- Primary Care experience is essential as you will be expected to run clinics and undertake home visits.
- GCSE grade A to C in English and Maths.
- Qualified to NVQ level 2 in Health and Social Care.
