Paramedic - South Warrington Primary Care Network

We are seeking an experienced and enthusiastic paramedic to join our team at South Warrington Primary Care Network. As a key member of our multidisciplinary team, you will play a vital role in delivering high-quality patient care and contributing to the development of our primary care services.
Key Responsibilities- Work collaboratively with our team to deliver patient-centered care, including face-to-face consultations, telephone triage, and home visits.
- Assess and prioritize patients, using your clinical expertise to provide accurate diagnoses and treatment plans.
- Develop and maintain effective relationships with patients, families, and carers, communicating complex information in a clear and compassionate manner.
- Contribute to the development of our primary care services, including the implementation of new clinical pathways and the evaluation of patient outcomes.
- Participate in regular team meetings and contribute to the identification of areas for improvement and the development of solutions.
- Qualified to degree/diploma level in Paramedicine or equivalent experience.
- Registered with the Health and Care Professions Council (HCPC).
- Completed two-year Consolidation of Learning period as a newly qualified paramedic.
- Three years' experience as a band 6 (or equivalent) paramedic.
- Commitment to working towards a masters level or equivalent capability in paramedic areas of practice within six months of commencement.
South Warrington Primary Care Network is one of five PCNs within Warrington, Cheshire. We provide primary care services to 51,763 patients, across five GP practices. Our team is committed to delivering high-quality patient care and contributing to the development of our primary care services.
We are part of Quay Primary Healthcare CIC, a non-profit company owned by the GP Practices in Warrington. Quay Primary Healthcare CIC provides business and practical support to our PCN and delivers additional healthcare services to our patients.
