About the Role

We are seeking a highly skilled and compassionate Children's Community Nurse to join our team at Kent Community Health NHS Foundation Trust. As a key member of our community children's nursing team, you will provide high-quality nursing care to children and young people with complex needs in their own homes and community settings.

Key Responsibilities
  • Provide holistic nursing care to children and young people with a range of complex needs, including palliative care, oncology, and respiratory conditions.
  • Work collaboratively with families, carers, and other healthcare professionals to develop and implement individualized care plans.
  • Conduct comprehensive assessments and provide expert advice on nursing care and management of complex conditions.
  • Develop and maintain strong relationships with community teams, including GPs, social services, and other healthcare providers.
  • Participate in multidisciplinary team meetings and contribute to the development of care pathways and policies.
Requirements
  • Registered Nurse (Child Branch) or RSCN with a minimum of 2-3 years post-registration clinical experience working with children and their families.
  • Proven experience of working in community settings, including palliative care and oncology.
  • Sound nursing knowledge and experience of all basic clinical nursing skills, including care of Tracheostomies, enteral feeding devices, and Central venous catheters.
  • Excellent communication and interpersonal skills, with the ability to work autonomously and as part of a team.
  • Ability to prioritize workload and manage competing demands in a fast-paced environment.
