About the Role

We are seeking a highly skilled and compassionate Children's Community Nurse to join our team at Kent Community Health NHS Foundation Trust. As a key member of our community children's nursing team, you will provide high-quality nursing care to children and young people with complex needs in their own homes, schools, and community settings.

Key Responsibilities
  • Provide comprehensive nursing care to children and young people with a range of complex needs, including palliative care, oncology, respiratory, and cardiac conditions.
  • Work collaboratively with families, carers, and other healthcare professionals to develop and implement individualized care plans.
  • Conduct thorough assessments, identify needs, and develop strategies to promote the health and well-being of children and young people.
  • Provide education and support to families and carers on managing complex conditions and promoting healthy lifestyles.
  • Participate in multidisciplinary team meetings to discuss patient care and develop strategies to improve outcomes.
Requirements
  • Registered Nurse (Child Branch) or RSCN qualification.
  • At least 2-3 years post-registration clinical experience working with children and their families.
  • Wide range of professional knowledge and experience of paediatric nursing, including palliative care, oncology, respiratory, cardiac, gastroenterology, special needs, complex care, and experience of working with children/young people with additional needs.
