Resident Care Management Assistant

We are currently recruiting for a Resident Care Management Assistant to join our team. This role involves providing administration support for the Social Prescribing Team and assisting with the coordination of Care Home MDTs.
About the RoleThe successful candidate will work closely with Care Home Leads and other primary care professionals within the PCN to complete administrative tasks for a caseload of care homes. You will also build and maintain effective relationships with GPs and primary care professionals to enhance the coordination of care for the residents.
Key Responsibilities- Support the organization with the delivery of Care Home MDTs within the PCN.
- Collate information to help individuals prepare for clinical discussions with primary care professionals.
You will receive a competitive salary of £25,500 - £31,000 per annum, depending on experience, along with generous annual leave entitlement and wrap-around professional and education support.
