**Job Responsibilities:** Primary Care Network Administrator
Role SummaryWe are seeking an experienced and skilled individual to fill the position of Primary Care Network Administrator. As a key member of our team, you will be responsible for providing administrative support to our Care Homes and Social Prescribing Team.
The successful candidate will coordinate care home meetings, facilitate weekly rounds with GP practices, and ensure timely assistance for staff and residents. This includes identifying residents requiring review or discussion, collating information to help individuals prepare for clinical discussions, and addressing changing needs.
Additionally, you will support the implementation of local priorities, address health inequalities, and promote population health. This role aims to improve communication between Primary Care and care home staff, reducing unnecessary GP visits and hospital admissions.
With a strong background in primary care administration, preferably in care homes or with care home staff/residents, you will bring valuable skills and expertise to our team.
