About the Role

We are seeking an enthusiastic and skilled nursing professional to join our Community Intervention Service team at Midlands Partnership NHS Foundation Trust. As a Community Nursing Sister, you will play a vital role in providing high-quality care to patients in their own homes, working closely with GPs and multidisciplinary teams to address a range of care issues and prevent avoidable hospital admissions.

Key Responsibilities
  • Deliver a high-quality service, working independently in a community setting to assess, plan, implement, and evaluate programmes of care.
  • Participate in the ongoing assessment, planning, implementation, and evaluation of care programmes, adhering to policies and procedures.
  • Work closely with GPs and multidisciplinary teams to address a range of care issues and prevent avoidable hospital admissions.
  • Support the Operational Lead and Clinical Lead in managing and coordinating aspects of care and service provision, including case management within the team.
Requirements
  • NMC registration and a qualified nursing practitioner with current registration.
  • Experience of working in the community, with a commitment to delivering high-quality targeted personal care.
  • A full driving licence.
