About the Role

We are seeking a dedicated and compassionate Adult Support Practitioner to join our team at Turning Point Scotland's Glasgow Mental Health & Huntington's Service. As a Support Practitioner, you will work with individuals who have a wide range of support needs, providing them with practical and emotional support and encouraging them to achieve their own personal outcomes in all aspects of their daily lives.

Key Responsibilities:

  • Provide individualised support to adults with complex and enduring mental health problems and individuals suffering from symptomatic Huntington's disease
  • Work with individuals to identify and achieve their personal outcomes, including keeping safe, meaningful activities, community involvement, physical health, relationships, emotional health and wellbeing
  • Develop and maintain positive relationships with individuals, their families and friends, and other health and social work professionals
  • Contribute to the development and implementation of individualised support plans
  • Participate in regular team meetings and contribute to the development of the service
