About the Role

We are seeking a dedicated and compassionate Adult Support Practitioner to join our team at Turning Point Scotland. As a Support Practitioner, you will work with individuals who have a wide range of support needs, providing them with practical and emotional support and encouraging them to achieve their own personal outcomes in all aspects of their daily lives.

Key Responsibilities:

  • Provide individualised support to adults with a wide range of needs, either on an individual 1:1 basis or as part of a team.
  • Work with individuals to identify and achieve their personal outcomes, including keeping safe, meaningful activities, community involvement, physical health, relationships, emotional health, and wellbeing.
  • Develop and maintain effective relationships with individuals, their families, and other professionals to ensure a holistic approach to support.
  • Contribute to the development and implementation of individual support plans, ensuring they are person-centred and meet the individual's needs.
  • Participate in regular team meetings and contribute to the development of the team's work.
