About the Role:

We are seeking a compassionate and caring individual to join our team as an Adult Support Practitioner. As a Support Practitioner, you will work with adults who have complex needs, providing them with practical and emotional support to achieve their personal outcomes.

Key Responsibilities:

  • Provide individualized support to adults with complex needs, including those with mental health problems and Huntington's disease
  • Work with individuals to identify and achieve their personal outcomes, such as keeping safe, meaningful activities, community involvement, physical health, relationships, and emotional health and wellbeing
  • Collaborate with other health and social work professionals, advocacy services, and family and friends to provide person-centered support
  • Develop and implement personalized support plans to meet the unique needs of each individual
  • Provide education, employability, and social activities through self-directed support
