Key Job Duties
  • Carefully conduct intake screenings for new inmates to pinpoint potential health hazards and create preliminary crisis management strategies.
  • Perform thorough health evaluations for all willing inmates within a week.
  • Take charge of identifying patients requiring reassessment following court or hospital visits.
  • Gather vital healthcare data from families and community service providers to enhance patient care continuity.
  • Implement prompt interventions and contribute to achieving performance targets.
  • Pursue short-term care coordination and intervene briefly in cases of substance misuse and mental wellbeing.
Essential Skills and Qualifications
  • Degree complemented by a diploma and specialized training.
  • Considerable clinical experience, encompassing individual and group work promoting social inclusion.
  • Documentary proof of eligibility to work, including identification and residency verification.
