**Job Summary:** Primary Care Mental Health Support Worker

**Estimated Salary:** £40,000 - £50,000 per annum

As a mental health practitioner working in this role, you will take a key role in developing and shaping a major new approach to providing mental health care within the community. You will work closely with General Practitioners and the primary care team to provide assessments, brief interventions, and care navigation for people with significant mental health issues living in the locality.

The successful candidate will need to work closely with local mental health services to support an easy in and out approach between secondary and primary care, ensuring seamless continuity of care for individuals and their families.
