We are seeking a talented Triage and Assessment Professional to join our expanding team at Swansea Bay University Health Board. As a key member of our service, you will be responsible for providing a single point of access for all referrals into Adult Mental Health services.

You will have the opportunity to work closely with colleagues to review referrals, identify gaps in the referral process, and strengthen existing joint partnership systems. The ideal candidate will possess substantial experience working within acute adult/community mental health settings, preferably with RMN registration or occupational therapy/social work qualifications. A postgraduate qualification or evidence of working towards one is also desirable.

In this role, you will be responsible for triaging referral information, making autonomous clinical judgments, and allocating tasks to the appropriate professionals/services. You will also conduct face-to-face assessments, formulate risk assessments, and provide brief interventions during telephone calls. We offer a competitive salary of £40,000 - £50,000 per annum, depending on experience.
