Social Care Professional

A qualified social worker is required to work within an integrated community mental health team in Caerphilly County Borough Council area.
Job Summary:The ideal candidate will have a relevant Social Work qualification, registration with Social Care Wales or Social Work England and at least 20 years' experience in the Social Work market.
Duties and Responsibilities:- To act as an assessor, reviewer and care coordinator for a caseload of individuals recovering from serious mental health issues.
- Participate in duty desk rotas, multi-disciplinary team meetings, assessment clinics and ward rounds as required.
The successful candidate will have access to good flexible working policies, regular supervision, a supportive team manager and free parking available on site.
Requirements:- A relevant Social Work Qualification
- Registration with Social Care Wales or Social Work England
- Enhanced DBS, registered on the update service or willing to apply for a DBS
- Driving license with access to a car
